Mississauga Halton Community Care Access Centre (CCAC)

The Mississauga Halton Community Care Access Centre (CCAC) is fully funded by the Ministry of Health and Long-term Care. Our main objective is to provide clinical assessments and manage the diverse needs of our clients, ensuring they can safely remain at home or transition to long-term care facilities. We cater to over one million residents in South Etobicoke, Mississauga, Oakville, Georgetown, Halton Hills, and Milton. Our services are funded by the Ministry, meaning there is no direct cost to clients. We specialize in caring for high-need clients and strive to help them stay at home for as long as possible. However, we may not be the best option for all seniors or citizens. Upon contacting us, you will undergo an assessment and be assigned a case manager or provided with information and referrals to alternative community services. Our main focus is to help individuals of all ages access the health and community care they require, whether it be at home, school, or within the community. We assess clients' clinical and support needs and deliver specialized healthcare services.
